All forms are sent directly to the owners.
The only way to get a response as to whether said form was received is to include one's e-mail address (or confront said persons in person).
Please give forms at least 24 hours to be received and read.
Please also give a week for your shirt to be made and returned.
Only students who go to Colonel By Secondary School may buy a shirt.
Friends of students may, if said friend will pay when shirt is delivered.
Pay is not needed until shirt is complete (and shown/approved), though shirt will not be handed over until payment is given.
