INSTRUCTIONS FOR FILLING THIS BOOKLET OUT ELECTRONICALLY
  1. Anything typed in the text boxes will not be saved. If information overflows the text boxes it will not print the overflow. If it does overflow, you must print your information seperately and attach it to this document. We suggest that you type the information in a word processor then cut and paste the information into the text boxes. In this manner you will have saved your work.
  2. You must print after you have completed the information, before you go on to the next page. If you do not the information will be lost and will need to be re-entered.
  3. Signatures cannot be done electronically, they must be done manually. (This concerns pages 5 and 6)
  4. Page one information can be typed in.
  5. Photos can be pasted in, or attached on a seperate sheet.
