Extra Credit Contract |

In order to have an opportunity to do extra credit assignments or projects at the end of the year, the student must fit all of the criteria below. |

| If student is PASSING at the start of the contract, student must: |
|
|
|
- Not have more than 4 HOMEWORK/CLASSWORK assignments that are either missing, incomplete, or late as of November 29th (1st sem) or April 23rd (2nd sem).
- Determined by my gradekeeping
- Not miss more than one assignment in any given week (absenteeism is not an acceptable excuse).
- Determined by my gradekeeping

If student is FAILING at the start of the contract, student must: |
|
|
|
- Not have more than 4 HOMEWORK/CLASSWORK assignments that are either missing, incomplete, or late as of November 29th (1st sem) or April 23rd (2nd sem).
- Determined by my gradekeeping
- Not miss more than one assignment in any given week (absenteeism is not an acceptable excuse).
- Determined by my gradekeeping
- Come for help once a week for at least 20 minutes (unless I decide that absence or vacation doesn�t permit it. Having a job or extra curricular activities is NOT an acceptable excuse to miss coming for help.)
- Come to point recovery for at least 3 tests

CRUCIAL NOTES |

|
- Needing this class to advance to another grade, or in order to graduate, does NOT exempt the student from any part of this contract.
- Fulfilling all of the requirements above and completing all of the extra credit does NOT guarantee that the student will pass or get the desired grade.

|
The contract must be signed by both the STUDENT and a PARENT/GUARDIAN.
Signing the contract does not require the student to fulfill the requirements, but only states that the student and parent/guardian have read, and understand, the requirements for extra credit eligibility. |
