| Madison Farms Adoption Contract I,_________________________________, wish to adopt an animal from Madison Farms Rescue and agree to the following Terms.. -I agree To provide past veterinary histort of my other pets, Including veterinary phone number and animals treated, Proof of spay & neutre of all presently owned pets must be provided. I understand that no animal will be placed in my home where there are unspayed Pets. - I agree to spay/Neutre all pets adopted no later then 4 months of age. -I understand that written proof of surgery must be handed to madison Farms Rescue no later than 30 days of surgery. -I understand that under no circumstances is this animal to be bred. I understand is the failure to spay/neutre this constitues a violation of the contract and then Madison Farms Rescue has the right to seiz the animal and assume full ownership of the said animal. -I will ensure that this pet will never be subjected to cruel or inhuman treatment, what constitues as inhuman treatment is to be determined by Madison Farms Rescue. -I agree to provide daily exercise in an enclosed area. -I understad that some pets will be placed with the stipulation that they are the only pets of that household.. -I understand that puppies can not be left alone for 3 consecutive hours while lhousebreaking training.. -I understand this pet must be kept primarly indoors(if cat dog or pocket pet).. -I agree to allow madison Farms Rescue to periodiclly inspect the property and where the pet is house to ensure propper care(Food, Water, Exercise, Champanionship and shelter) -I understand Madison Farms Rescue will be contacting my veterinarian for vefication of routain preventative care needed in my specific area (Special diet, Hartworm/Flea/intestinal parsite prevention) -I understand that I will be required to return this pet to Madison Farms Rescue if I am in violation of all or part of this contract. -I will Contact Madison Farms Rescue Immediatly if my adoption in not successful, And returen this pet to Madison Farms Rescue. I understand this pet is not to be sold/ given away or placed in another permanate home or organization.. -I agree to notify Madison Farms Rescue If I have a change of address, or any change in the circumstances of the animal (loss,theft seriouse injury or illness. -I have read all terms or this contract and I understand that Madison Farms Rescue can make no gureentees on temperment or future health of this animal. I HEARBY AGREE to realease Madison Farms Rescue from all liabilty for damage or injury which may be caused directly or indirectly by thi pet. I understaand my new pet my not be housebroken/Crate trained.. -Adopter is responsible for all legal costs incurred by madison farms rescue to enforce this contract and agree to all the above terms of this contract.
