Oak Hill Cemetery, 444 Oak Hill Road, Youngstown, Ohio
The headstone for Caleb Baldwin is located in the Wick section of the cemetery.  It is most likely that he was buried in the original cemetery downtown and was moved to this cemetery when it opened.  According to the Mahoning Valley Historical Society the Daughters of the American Revolution sponsored a new headstone for Caleb Baldwin in 1903, so this stone is probably not the original.
It is extremely worn, and it was difficult to read the inscription but here is what I could decipher:
In Memory Of
Caleb Baldwin
Who depated this life February 19, 1810
He loved the Lord, beloved God
--the rest was too worn to read
