Moon Prism Power, Make-Up! ~ Her original transformation used during the first season and part of SMR. Holding her locket above her head Usagi shouts, "Moon Prism Power, Make-Up!" Her clothes dissapear and a shimmering light are in their place. Pink ribbons shoot out from her locket and wrap around her body forming the body suit. The ribbons form her gloves and her boots and the her skirt with the bow in the back. Her tiara and earrings form in a sparkle across her forehead and she poses with the moon behind her.

Moon Crystal Power, Make-Up! ~ Usagi's second transformation during SMR and part of SMS. It is very similar to her first transformation except for three things: angel wings cover her body at the beginning, the locket now holds the ginzuishou (silver crystal), and a crescent moon shines on her forehead before being covered by the tiara. The moon behind her now has colored flowers decorating it.

Moon Cosmic Power, Make-Up! ~ Usagi's transformation for most of SMS. Holding out her compact, she shouts this phrase and is covered in light. A large blue heart spins around her body. As she spins her gloves, boots and bodice forms. The heart grows smaller and becomes her skirt. The crescent moon on her forehead flashes and the tiara appears with a moon at its center. She poses with a large pink heart behind her.

Crisis Make-Up! ~ Usagi's transformation into Super Sailor Moon. She holds up the Holy Grail and shouts this phrase and is enveloped in hundreds of butterflies. Her skirt becomes white with a band of color at the hem and her back ribbon gets longer. Transparent sleeves now cover her shoulders and white feather pins appear above her bangs. She poses with a large pastel tie-died butterfly behind her.

Moon Crisis Make-Up! ~ Chibi-Usa and Usagi transform together in Sailor Moon SuperS. With power from Pegasus they become Super Sailor Chibimoon and Super Sailor Moon.

Moon Eternal Make-Up! ~ Usagi's final transformation in the fifth season, Sailor Moon Sailor Stars. The cover of her compact dissapears as she yells. She passes her hand over the crystal and it blinks. A gold moon appears on her forehead and white-pink energy covers her body. Wings appear on her back. She place the compact on her chest and spins. Her bodysuit appears. Gloves appears as she extends her arms, left and then right. Turning back to face forward, her boots then her skirt appear. With a final turn her wings are modified and she is Eternal Sailor Moon.
