Objective: students will be able to create forms using the form wizard.

1. Start>programs>Access>open an existing database>OK>file name: dbSurvey.mdb>open
2. Form>new>Form wizard>tblCustomers
3. Select all in Available Fields>next
4. Columnar>next
5. Standard>next
6. Modify the form's design>finish
7. Form>new>Form wizard>tblRestaurants
8. Select all in Available Fields>next
9. Columnar>next
10. Standard>next
11. Modify the form's design>finish
