| I, _____________________________________, parent of ______________________________, hereby give my permission for my child to attend book club meetings at the home of Sabrina Camacho on the first Tuesday of each month for the entire 2007-2008 school year (there are no meetings in June or July). I understand I am to have my child at Sabrina Camacho's home by 12:00pm on meeting days and have my child picked up by 1:30pm, unless I have made prior arrangements. I further understand my child may provide his/her own drink and lunch to eat at the meeting.
