| I, ______________________________________, parent of _________________________________, hereby give my permission for my student to attend book club meetings at the home of Sabrina Camacho, ride with Mrs. Camacho or her assigns to attend REACH High Student Council meetings at the Irmo Branch Library immediately after lunch on the dates noted below. All students should bring $5.00 and a drink to the meeting if they wish to eat before the Student Council meeting. We will order pizza to be eaten here. |

| I understand I am to have my child to Sabrina Camacho's home by 11:30am on the above dates and have my child picked up by 12:30pm unless they are purchasing pizza to eat at the Camacho's. My student will be transported and I am to pick my student up at the Irmo Library by 2:45pm on the above dates. I further understand that my student will be unchaperoned after 2:45pm, and I absolve Sabrina Camacho, or any other member of REACH, of any responsibility for my student after that time.
