Maiden/Former Name Report

This FOCEXEC will list the students who have an outstanding high school transcript and who has listed a maiden/former name on their application. There are times when the admissions office will have a copy of a student's high school diploma that lists their former name, but their new name is the one listed in SIS. Naturally, no type of student id will be listed on the diploma. So this report will provide the link between the two. This assumes the admissions office "captures" former names in SIS.
