Instructions: Microsoft Word 2000 Documents can be downloaded by participants, filled out on a computer and then printed. Or, they can be download to a server, locked, and then used by participants. Print hard copies of PDF documents and fill out the hard copy. Return all computed forms to Phoenix Think Tank, 2411 West 14th Street, Tempe, Arizona 85281.
