The process of this study has been explained to me and I have given full permission for this study to be conducted. SJP is also granted permission to access and review documents and public records related to that location's history. Permission to access the location on specific days and times will be provided as verbal agreement(s) between SJP and myself. I understand that I may be present at the location when this study is being conducted, and I do not relinquish my authority over this location in any way. SJP releases the Trustee from any liability for injuries that might occur during the study. In addition, the participating members of SJP assume responsibility for any damages caused to the location's physical property that occur due to member(s) actions. SJP will also inform the Trustee of any hazards encountered at the location during the course of the investigation. SJP also assumes responsibility for releases of information about the location, the Trustee, and associated parties. No information about the investigation will be released without signed permission of the Trustee, and only to an extent agreed upon in writing.
