Periodontal Diseases: Current Concept Management & Research
Prof. L. T. Hou 侯連團 Chair of Periodontology Dept., National Taiwan University, Taipei, Taiwan

摘要:
Periodontal diseases are complex disease entities affecting the support structure surrounding the teeth, the only masticatory apparatus in human being. Without proper prevention and treatment, the progression of the disease processes could laed to excessive tooth mobility and eventual loss of involved teeth. The diseases are mainly caused by many virulent oral microorganisms resident inside the bacterial plaque accumulated around the cervical portions of teeth. These bacteria are known to prod many toxins, such as endotoxin or exotoxin, act as invading antigens which trigger immune responses and induce destructive inflammatory reaction of the gingival (gum), also release many enzymes to digest of degrade tissue components around the teeth. Moreover, the release and leakage of cytokines (IL-1B, TNF-a, IL-2, IL-6, IL-8 etc.) elaborated by inflammatory cells (macrophage, neutrophils etc.), into gingival tissue in addition to causing bone resorption, could augment the synthesis of tissue degradation enzymes by resident cells stimulated. The latter changes further aggravate the periodontal tissue destruction. Although the outcome of uncontrolled periodontal diseases is disastrous, the diseases can be easily prevented and the results of early detection and treatment are quite successful. In my presentation, the main etiology well as common symptoms and signs of the periodontal diseases are discussed. Furthermore, current concepts on disease prevention and surgical management of the diseases are addressed.
