Release of Child
1. You will need to provide a list of those who are authorized to pick up your child, and verbal notification whenever anyone on that list (besides yourself) will be picking your child up. A driver's license or photo ID will be required. Under no circumstances will a child be released without notification and identification.
2. If anyone, including the parents/guardians, arrive to pick up the child and seem to be under the influence of drugs or alcohol, an alternative means of transporting the child home safely must be found. If refused, while I cannot legally prevent the parents/guardians from taking the child, in such an event, I will notify the police and provide them with the license plate number of the offensive vehicle. |

Participation, Transportation, and Substitute Care
1. This policy grants permission for your child to play on all indoor and outdoor play equipment, and to participate in all activities of my childcare home. This policy also grants permission for me to take photos of your child during daycare or on field trips. I will provide duplicate copies to you at your request and expense. (Most photos will be taken with a digital camera. If you choose to provide me with your e-mail address, I will happily forward photos to you!) The photos will be used in my childcare home only and I will approve any other use first.
2. This policy grants permission for me to take your child off the premises on foot or in a vehicle. Your child will be placed in an appropriate car safety seat as required by law.
3. This policy authorizes me to leave your child in the care of a substitute care provider of my choosing should the need arise. |

Termination
1. You are require to provide a minimum of two-weeks advance notice of termination. Childcare fees for the duration of this notice will be due at the time of notice, and are required regardless of whether or not the child attends for the duration of the notice.
2. EXCEPTION: The first week of care will be considered probationary for provider, parent, and child. The childcare contract may be terminated at any time during this probationary period without the requirement of a two-week notice and the accompanying fees.
