Online OH-1 Crash Form Browser - Search utility intended to allow specific users access to a scanned image of the paper form for the corresponding record.

Injury Surveillance System Quarterly Reports - Developed a report library of over thirty reports from scratch. Utilized bind variables and built-in report triggers to automate report generation as PDF documents. Approximately ten reports include dymanic graphs and bar charts developed with Oracle Graphics 6i.
Ohio Crash System Reports - Created reports to support the county engineers interpret crash data. Used parameter form to futher define queries at runtime. These reports were designed to be run using an Oracle Portal.
Highway Safety Exposure Data Warehouse Reports - Designed a library of fact sheets to report the exposure of national, state, and countywide crash data. Reports utilize a variety of features, including summary columns, and matrix groups.

Hospital Trauma Data Export Utility - Stand alone freeware application distributed to clients to assist in exportation of Trauma data.
Cales2TSASS Data Extract Utility - Conversion utility used to extract essential data elements from certain client's third-party software.
Ohio Crash Data Entry - Stand alone freeware application intended to allow Ohio officiers to enter crash forms at the scene of an accident using a labtop computer.
Census Data Extract Utility - Translation utility used to convert specific formatted files into an Oracle Loader file.

Traffic Records System Inventory - A collection of each US State and Territories traffic data systems contact, as well as some general information about each state's system. This database powers the Traffic Records System Inventory website which is maintained for the federal goverment and otehr safety professionals. Database was designed based upon a paper survery, and implemented from scratch. Several macros were developed to help facilitate validation of recently submitted data.
EMS Incident Reporting System - Freeware Access application which allow EMS Run Reporting Facilities to enter, retrieve, and export data to the Ohio Department of Public Safety.
Safe Communities Citation Data Reporting - In-house application used for entering citation data for Medina County and other Safe Communitity Participates.
Hazardous materials RISK Simulation - Access application used to calculate the risk invovled in shipping hazardous materials through Ohio. Allowed user to enter a proposed route and returned the associated risk factor.
Golden Christian Academy Student Grade Database - Used by GCA to track students, teachers, courses, and grades. Includes the ablility to print report cards, transcripts, and honor roll certificates.
Glenville High School Senior Parents Group Contact Database - Used by GSPG to contact parents of the Glenville 2002 senior class. The GSPG used the database to help raise funds for the Class of 2002.
Web Based Programming
Traffic Records System Inventory - Designed a web interface to allows traffic safety professionals to browse the systems of various states in the Traffic Records Systems Inventory database. Allows users to alter data systems, with password protection for certain users.
Safe Communities Citation Data Reporting - Created rollover map navigation for the Medina County Safe Communities page using JavaScript. Created a separate report navigation image using Flash. Generated HTML reports using Excel to display citation, warning, seat belt usage, and EMS Run data for Medina County Safe Communities program.
Highway Safety Exposure Data Warehouse - Create basic website which dynamically displays available exposure reports for a given jurisdiction. Also created a simple querying interface which allows users to select specific data directly an Oracle database.
TSASS Internal Contacts Database Web Interface - In-house web application which allows off site employees to browse and update contacts from the company database.
