USMLE

Zkoušky USMLE jsou administrovány pro zahraniční absolventy lékařských fakult ECFMG - Educational Commision for Foreign Medical Graduates sídlící v Philadelphii. Pro získání licence praktikovat medicínu v USA je nutno složit několik zkoušek. Takže postupně. První je STEP 1 of USMLE, což je zkouška z preklinických oborů tzv. Basic Sciences. Druhá je STEP 2 of USMLE a tato zkouška je zaměřena na klinické znalosti a uplatnění diferenciální diagnozy. Další zkouškou je CSA jediná zkouška ústní v sérii testů. Skládá se v nemocnici ve Philadelphii a zkoušený zde dostane devět standardizovaných pacientů, kde pak má na každého pacienta půl hodiny na to aby odebral anamnézu a vypracoval diagnostický postup s návrhem léčby a diferenciální diagnózou. Kromě odborných znalostí je zde kladen důraz také na schopnost komunikace v anglickém jazyce. Posledním z této série testů je STEP 3 of USMLE, kde je testována znalost z urgentní medicíny plus aplikace znalostí terapie a managementu lékařské péče. Mimo tyto specifciky medicínské testy je také nutno složit zkoušku TOEFL - Test of English as a Foreign Language. Většina z těcho testů se v současné době provádí na počítaci CBT - Computer Basted Testing. Jediná zkouška, která se neprovádí na počítači je tzv. CSA - Clinical Skills Assesment, která je ústní přímo v nemocnici. Provádí se pouze v Philadephii.

Ceny zkoušek se pohybují takto:

There are approximately 1,400 medical schools throughout the world, each with varying educational standards and curricula. The purpose of ECFMG certification is to assess the readiness of graduates of foreign medical schools to enter U.S. residency and fellowship programs that are accredited by the Accreditation Council for Graduate Medical Education (ACGME).

If you are a foreign medical graduate and wish to enter an ACGME-accredited residency or fellowship program in the U.S., you must be certified by ECFMG before you can enter the program. You must also be certified by ECFMG if you wish to take Step 3 of the three-step United States Medical Licensing Examination (USMLE). Most states in the United States also require ECFMG certification to obtain licensure to practice medicine.

To be certified by ECFMG, you must pass a series of exams; you must also provide ECFMG with copies of your medical diploma, which ECFMG will verify directly with your medical school (see Requirements for Certification at left). The certification process begins when you submit your first application for examination to ECFMG. At this time, ECFMG will assign you an identification number that you will use throughout the certification process.

The time required to complete the certification process is different for each applicant. There is no minimum time period required for ECFMG certification. You may apply for the required exams as soon as you are eligible. All of the exams required for ECFMG certification are offered continuously throughout the year.

Both medical school students and graduates may begin the certification process. However, since one of the requirements for ECFMG certification is that your medical diploma be independently verified by your medical school to ECFMG, you cannot complete the certification process until you graduate and receive your diploma. The time required for some aspects of the certification process, such as the time required by your medical school to verify your medical diploma, are beyond the control of ECFMG.
