I host my domains at ZoneEdit.com and think they offer an excellent service. They offer support even on the first 5 free zones and in general have a very functional and usable interface for maintain your own zone file(s). As additional secondary DNS I use secondary.com. They also have an excellent primer which I use to explain to clients (that want to know!) how DNS works.

Here's an example of a pdf that was created to allow filling in a form. It's a name reservation form for a South African Close Corporation.
