666 West, Morganfield, KY 42437, Hereinafter referred to as "Seller" for the purchase
described below:

PURCHASE PRICE;
For the total sum of ___________________ plus any shipping costs, the Seller agrees to
sale and the Buyer agrees to buy said puppy based upon the following terms.
PAYMENT TERMS
The buyer agrees to pay $150.00 , as a non-refundable deposit for said puppy by mail
(postal money order) with a balance due and payable by the time the puppy reaches 14
weeks of age . If seller should not receive Full purchase price plus shipping if applicable
by this time, Seller is not liable for any refunds to Buyer. If for some unforseen reason
the puppy is not made available to the buyer by the puppies 14 th week the Buyer is entitled
to a full refund of all monies.
