5, 10, and 15 years of service in New Mexico; plaques are awarded for 20 and 25 years of service.  A
special  award will be made when an active professional reaches 30 years of service.  Please be reminded
that our State Association was incorporated in 1983; 2004 will be the 21st year for our state association.
The following application will take about 2 minutes to complete and no other information is required for
application for length of service awards.
