@Memory not leak checked
After allowing the whole sample to play if the next sample is not
occupied, it does not correctly stop the note sometimes, until the note is resampled.

Pattern gets damaged and needs full rewrite for unknown reasons sometimes
Sometimes fails to start in Jack mode, some initialization out of order in the threads possibly

Sometimes stop/start does not restart the sound
    Note, this is likely due to the order of stopping the stream vs. closing the stream
    Does not misbehave on my desktop but it does on the laptop -- depend on sound module or alsa/oss?

Initial load of samples -- should go to pitch 0 ( not -1 ) Is -1 needed for any reason?

Panning causes distortion
   Choose a different function relating the setting to the actual volume, so that adjusting away from
   the center does not increse either volume above the initial value.

Error handling for portaudio stream
   also see above bug re stop/start of stream
