| Abstract: State the primary focus of the program and identify the issues/areas it was designed to address. Utilizing the categories listed below as headings, provide the following background information: � Community demographics such as urban, rural, ethnicity, and level of involvement in school activities (In Rhode Island, use of InformationWorks! and district and/or school level SALT survey data is encouraged and will be used as the basis for validation); � School setting including: goals and philosophy; school and family and consumer sciences enrollment; and types of extracurricular activities offered; � Program restrictions because of federal or state regulations and/or school policy such as enrollment, class size, scheduling, program budget, and funding. PART 4 Use a maximum of one (1) typed, double-spaced page to describe the specific goals(s) and list the intended outcomes of the nominee�s educational program. Include only the information about the program for which the nominee is being nominated. PART 5 Describe the nominee�s educational program. Reminder: A maximum of 14 typed, double-spaced one sided pages ( or seven (7) double sided) can be submitted for this response. Photographs may be used to help illustrate and explain the program, but must be counted as part of the 14 pages. Using the categories listed below as headings, provide information on or give evidence of the following in narrative form: I. Attainment of the program goals and intended outcomes. II. Enrollment data, and evidence that the program is sustainable, and can be replicated. III. Creativity, innovation, progressive approach to subject matter. IV. Ingenuity and perseverance used to obtain, maximize, and sustain funding and/or overcome obstacles of the program. V. Integration of family and consumer sciences concepts with subject matter focus, and with other disciplines. VI. Influence on students� lives within and beyond the program. VII. Relevant and timely impact on students, school and/or community. VIII. Increased visibility, recognition, appreciation, and support of the profession. PART 6 The nominee may provide up to five letters of support, which provide evidence of the impact and importance of the nominee�s contributions to students, families, and the community, such as those from supervisors, colleagues, parents, students or community leaders. In lieu of, or in addition to letters of support, members of the awards committee may visit the program. |
