| Adoption Contract Date: Name: Address: Phone: Animal Name: Do you agree to the following Yes or No (put a yes or no at end of sentence.) 1: I can provide proper care for the animal that I am adopting and If there is ever a time when I cannot I agree to bring the animal back (without refund). 2: I know basic information about the animal I want to adopt and know how to take care of and house the animal properly and if I do not I will ask any questions possible to assure I learn. 3: I will read through the care sheet provided at least one week before I adopt the animal even if I think I know how to take care of the animal to be on the safe side. 4: I will not mistreat/misuse the animal in anyway or I am aware that I can be fined and prosecuted if I do (RCW 9.08.070 Injuring/Killing ect. Any person who does any of the following shall be guilty of a gross misdemeanor punishable according to the chapter 9A.20 RCW and by a mandatory fine of no less than five hundred dollars per pet animal, For Willfully or recklessly kills of injures any pet animal.) 5: I have a job and or money to provide a proper home. 6: I am at least 18 years of age with proof of id. 7: I already have the cage and supplies needed for the animal I want to adopt OR I will by the time I bring it home. |
