Catastrophe - Visit North Corel once Meteor has been summoned and talk to the lady in the middle house on the west side of town. She gives it to you hoping you can use it. |

Limited Moon - After taking Bugenhagen to the Ancient City, return to Cosmo Canyon with Red XIII in your party. Find Bugenhagen very ill in his observatory and talk to him. |
Cosmo Memory - You must defeat Lost Number in the Shinra Mansion (which requires you to open the safe on the second floor of the mansion). |

Princess Guard - While in the Temple of the Ancients, you'll find this weapon. It's at the end of the Clock Room's IV hallway (though I don't think it's an "ultimate" weapon because you can grow materia in it, it doesn't have a full row of Materia slots, and it's not even the strongest [but the Parasol has NO Materia slots]). |
Great Gospel - Speak to the sleeping man near Junon when the last two digits of the number of battles you've fought match and he will give you a piece of Mythril. You must then take this to the Weapon Seller (East of Gongaga), give it to him, and choose the small box (*NOTE: There's no real point to doing this unless you have Gameshark and are able to bring Aeris back because it's impossible to have Aeris and the Limit Break at the same time). |

Venus Gospel - After the rocket in Rocket Town is launched, talk to the old man outside the Item Shop. If you speak with him several times he'll eventually give it to you. |
Highwind - Get inside the Crashed Gelnika, locate it in the cargo bay. If i remember correctly, it's on the lower level. |

Conformer - After aquiring the submarine, take it to the crashed Gelnika (near the Gold Saucer); it's in the Generator room (search thouroughly in the areas where you can barely see Cloud). |
All Creation - Defeat Godo at the top of the Pagoda in Wutai. |

Death Penalty/Chaos - After gaining the submarine/Gold Chocobo, you can reach Lucrecia's Cave. Visit once during Disc 2 and visit again during Disc 3 (Vincent recieves his weapon and limit break on the second visit). |

HP Shout - During the team's second raid on Shinra's Headquarters, you can find it in one of the lockers on the 64th floor (on the left side of the third row). |
