PROSPECTIVE MEMBER |

| 1. I am aware that the Redwood City Seņors
Softball Club is a non-profit organization that operates under a set of |
|
| by-laws
that those by-laws may be inspected by me at any reasonable time by
contacting the Club Secretary. |
|
|
| 2. I recognize that it will be my
responsibility, as a member, to actively support and participate in the
activities of this |
|
| club
requested, to provide reasonable assistance in conducting these activities,
(i.e. games, functions, etc.). |
|
|
| 3. I agree that neither the RWCSS Club, Inc.
nor its elected officers are liable for any injuries that I may incur through |
|
| my
participation or in related club activities and I agree that I am, and will
remain, covered by some form of medical |
|
| insurance
while I am a member of this club. |
|
|
| 4. As a member of the RWCSS Club, Inc., I
agree to conduct myself respectably at all times and that when participating |
| in
activities, I will: |

a.
Abide by the by-laws of the club and the rules and decisions of the
club officials. |
|
|
b.
Accept the decisions of the umpires and my team manager. |
|
|
c.
Avoid bodily contact that may cause injury to myself or others. |
|
|
d.
Refrain from publicly degrading fellow club members. |
|
|
e.
Never direct abusive or profane language at officials or fellow club
members. |
|
|
f.
Control the use of alcoholic beverages so as not to offend anyone or
to cause criticism of the club. |
|
|
| I am fully
aware that the rules and code of conduct of this club will be enforced and
that behavior on my part, that |
|
| violates
the rules and code, may be cause for restricting my participation in certain
club activities and/or terminating |
|
| my
membership. |
