The lady is a tight-rope walker and is doing the stunt blind-folded. When the music stops it means she is safely across and can step over to the platform, but someone turned off the music early, and she falls off the rope to her death.
They were standing under a McDonald's sign, the letter M. The "M" fell down, and its three points killed three of the men. The other two were saved because they were standing under the humps.
The ambulance was carrying donated blood. The car hit the ambulance, and the ambulance doors flew open. The bags of blood broke and spilled.
There had been four men on the boat, and only three lifejackets on the hooks. There was a storm, and when it was evident that the boat would be wrecked, the men played cards to see who would not get a lifejacket. The loser was the dead man. The other three survived.
There were poisoned ice cubes in the lemonade. The first man drank quickly, and finished his drink before the ice cubes melted. The second man drank slowly, the ice cubes in his mug melted, and he was poisoned as he drank the last of his drink.
