The number 1 thing to remember when dealing with an opponent with a knife: YOU ARE GOING TO GET CUT. These techniques will allow you to escape with your life and minimize the damage done to you.
Knife Defense
Straight In (orange belt)
The key to this defense is to move off line and avoid the stab by turning your body away from the knife while delivering a strong knife hand block to the wrist withright hand.

Left knife hand to bicep (hand in line with his arm). Roll/slide hand down, covering while delivering right knife hand to side of neck.

Behind Back (purple belt)
Glance behind back to see which hand the attacker is holding the knife in. You must learn BOTH sides.

The key is the lock. Turn in direction that he is holding (if he is holding left hand, turn to your left). using your arm to hit his hand away, then pinning his arm against his chest. You should be close to him, your hand on his opposite shoulder, his arm bent and pinned against him.

Use the finger under nose, thumb under jaw to lock his spine. Hawk down with right leg.
Knife to throat from behind (blue belt)
In one VERY QUICK MOTION, lock chin down on hand while bringing right hand up to pin his knife hand. Immediately bring left hand up to grab also (I am assuming he is using his right hand for simplisity sake. However you do need to learn this from both sides).

Stomp to soften on his attacking side.
Back step behind him, bringing your head down (KEEP BACK STRAIGHT) and spinning out of his grasp (STAY CLOSE)
In the perfect situation the knife will go in his side as you come around, opening him up.

Twist and lock his arm behind him and very quickly come down with a right downward elbow on his elbow, breaking the arm.
From that drop right hand down to grasp his wrist still maintaining the lock while you let go with your left and  palm strike the back of his knife hand, making his release the weapon.
Back to a wall, knife to front of neck (green belt)
Assuming, again, a right handed attack. Bring your left hand up suddenly to pin his wrist against your chest. At the same time bring your chin down so as to reduce potential damage (this is not a trap with the chin, just a safety precaution).  A split second after you move left hand bring right up to do a forearmstrike on his forearm and smash his throat as in lapel grab...

A) roll hand around neck and bring to front headlock while driving knee into stomache. Drive his head backwards into wall.

B) deliver a forearm strike to the outside of his elbow. You will need to keep your arm nearly straight and sweep it in a couter clockwise motion to do this. He should turn to your right. Drive a side blade into his knee but DO NOT recover. Keep it behind the joint as he goes down. Rake his face with a tiger claw, puching him backward and popping his knee out of joint.
