Knife to throat from behind--lock chin down on hand while bringing both hands up to grab his hand. Don't let go. Stomp on his foot to soften on his attacking side. Back step behind him, staying close, bringing you head out of his grasp. In the perfect situation, the knife will go in his side as you come around, opening him up. With your right arm, use a downward elbow to break his arm. Cup his hand holding knife with your right hand, and strike his knuckles with your left hand, causing the knife to drop.
