1. 以劃線支票及抬頭請註明“FHKPUAA”，並連同入表格寄回香港理工大學校友會聯會。
(By crossed CHEQUE, it should be made payable to “FHKPUAA” and be posted to FHKPUAA with Application Form.)
2. 銀行入數或櫃員機轉賬(恒生銀行帳號：296-069-339-668)，請保留銀行收款記錄副本，並連同入表格寄回或傳真致香港理工大學校友會聯會。 (By bank cash deposit or ATM (HSB Account No.: 296-069-339-668). Please keep a copy of the payment record and post or fax it to FHKPUAA with Application Form.)
