Consent Form
Description: Your participation is requested for this experiment in which several factors will be examined through the utilization of an intelligence test and personality test. The personality test consists of 44 questions and will take about 3 minutes to complete and IQ test will take approximately 12 minutes and involves 38 multiple choice questions. General information will also be requested in the form of a short questionnaire. Shannon Rosenbaum, a psychology student at TAMUK, is conducting this experiment.
Risks and benefits: There are no risks of any kind associated with this experiment. Benefits will arise in the establishment of a better understanding in areas including intelligence and personality types.
Confidentiality: All tests and information will be kept anonymous. Confidentiality is guaranteed and only the experimenter will have access to the information. With this in mind, it is requested that you be as honest and possible and try your best.
By signing this, I agree to the terms listed above and give my consent to participate in this study. I also understand that this is done on a volunteer basis and I may withdraw my participation at any time. If there are any questions I may have, I can contact Shannon ([email protected]).
