Form Submission system

ProAcumen Ltd have designed and developed a sophisticated Forms Submission system that uses the Internet to manage the whole process of setting up and gathering electronic questionnaires, forms or surveys.

Our system offers many quantifiable advantages:

  1. Simplicity in designing - and better still - editing questionnaires. Individual questions can be added or deleted long after a traditional paper questionnaire has been fixed and printed.
  2. No manual keying of responses from paper Forms.
  3. The data gathered can immediately be exported to other databases/systems.
  4. The system (instead of the User) carries out any mathematical calculations (i.e. totaling amounts or working out discounts etc).
  5. Users only see questions depending on previous answers they have given. This means Users are not daunted by larger questionnaires when they only need to complete a small part of it.
  6. The whole process is cheaper. There are no paper and postage costs and set-up overhead is greatly reduced.
  7. The whole process is simpler to manage.
  8. Speed. From commission to results - our system speeds up the whole process.
Functionality

The User

Users have all the functionality they expect when using a web-based system:

  • Registration
  • Password remind
  • Edit User details

When logged-in, Users see a Control Panel of all Forms (Questionnaires/Suveys) available to them and those that they are working on. Users can start Forms, save them, and come back to them at any time later to finish them off.

Forms that are in the process of being filled-out can only be seen by the User. When a Form is submitted, it can then only be viewed (i.e. not edited) by the User. At this point the Form becomes visible to the Administrator.

If there are mistakes on the submitted Form, the Administrator can "Return" the Form electronically to the User in seconds. The User can then re-edit and re-submit the Form at their leisure.

The Administrator

Administrators have complete control over the system. They can:

  • See all new registrants
    • Provide Usernames, Passwords and any other User control information.
    • Enable/Disable User accounts.
  • See all Forms submitted.
    • Choose to Acknowledge and/or Return them.
  • Create a new questionnaire
    • Add questions to a questionnaire - or edit/delete questions.
  • Use the submitted data in any number of ways.
  • Search the whole system.
  • See Management Reports that give data on all aspects of the questionnaire progress (i.e. between dates X and Y, so many Forms have been started but not completed, so many have been submitted etc).
  • For Users who submit many forms, there is Batch Import functionality that allows the automatic creation of Forms based on data submitted in an electronic file.

Our system is built around the principle that a questionnaire is not a set entity. It is built (every time it is accessed!) using individual questions - themselves each stored in the database.

A question itself is made up of various parts: the question name/number, question text, input boxes, question logic (what happens to that data, calculations, which question to jump to next) and so on.
