Fill out the Submission Form online or fill it out and print it out. All film submissions should be dropped off at the Madstone Theater at San Mateo & Academy on the submission date by 10:30pm.
By filling out this form, I acknowledge that all of the information on this form is correct and that my film, or portions of my film may be used in future screenings, on compilation tapes/DVDs, or in promotional materials. PMF, nor any of its members will be held responsible for any of the content of any of the films, and I as the filmmaker am responsible for all of the content of my film. Any films containing any copyrighted materials which have proper copyright clearance may be shown at PMF screenings and/or put on tapes/DVDs, and a copy of the copyright clearance shoulud be included with the submission form. Filmmakers should also obtain releases for anyone or any locations used in their films, but these do not need to be submitted.
If the film being submitted has more actors or characters than will fit above, submit a second form with the title of your film and "Addition" on the end. None of the other information needs to be filled out other than cast.
