Thank you for choosing PMC. In order to serve you better at your first appointment, we ask that you BRING WITH YOU as much of your pertinent medical information as possible. This means X-ray, MRI, CT, EMG, etc. and reports. We may need records from your family physician, other specialists, and other facilities. So, there will not be delays, plan in advance to obtain your records from them. We will start by getting to know you through your medical history. It is important that you list all of your current medicines, and/or bring them with you to your first appointment. With the necessary information gathered and through examination, we can device a treatment plan with you. Also, please bring your insurance cards for filing claims. We request a 24-hour notice if an appointment must be changed so that we can better serve you and others. We appreciate the opportunity to serve you.
