Potential Risks
The human subjects who participate in The Memory Game will not be harmed in any manner, due to the fact that the Memory Game will not contain any potential risks. However there are some health hazards that the human subject should be aware of. If a human subject has a headache, migraine or any types of head problems/ eye problems, that subject should not be allowed to participate in the Memory Game, due to the flashing objects that appear. A human subject is not allowed to participate in the Memory Game if their doctor or physician tells them not to do so. Overall, the Memory Game is harmless and will not effect a human subject physically, physiologically, socially, legally or in any other way. The Memory Game will only challenge the mind, but not effect it.
Other Procedures
The procedures that are involved to play in The Memory Game are written in the consent of the human subject. The procedures that would be used to minimize risk are that I would not allow anybody who has had many minor or major head problems prior to the experiment, participate in the Memory Game. The information of the human subject will remain confidential. This means that the score and age of the human subject will also be kept confidential to the public. This means that if the individual chooses to share their scores with other, it is within their full power. It would not be a compulsory for any human subject to participate in the Memory Game if they do not wish to do so. If applicable, prior to participating the Memory Game the human subject must complete a portion of the Human Subjects Form (4).
