Virtually everyone's died in Farscape once or twice. So here's where I get to tell you who's killed who, how, why, and what was done (if anything) to alleviate the problem.

Aeryn = 2 deaths

 Episode 2.16 - The Locket
Died from old age. However, Moya Starbursted backwards which managed to reverse time and so she was brought back to life. Only Zhaan and Stark knew about her death after this (everyone else forgot because of moving back through time) and stopped the flow of events which would lead to her death.
 Episode 2.22 - Die Me A Dichotomy
Crichton, under the control of Scorpius's neural clone, landed his module on top of Aeryn's Prowler. Aeryn ejected, but her seat harness was jammed. The seat's jets weakened the ice of a frozen lake and, pulled down by the chair, she drowned in it. However, Zhaan gave her some of her life energy in order to resurrect her, leading to Zhaan's near-death condition later (it should be noted that it was not the drain on Zhaan from resurrecting Aeryn that killed her).

Chiana = 2 deaths

 Episode 2.06 - Picture if you Will
A portrait foretold her burning to cinders and, believe it or not, it happened. Thing is she was in a freezer at the time. This was remedied because it was all a plot of Maldis's and she was taken to Maldis's realm, which Zhaan managed to get everyone to escape from.
 Episode 3.06 - Eat Me
Trapped on Rovhu with Crichton, D'Argo and Jool, Chiana was twinned by the insane Karvok. And then had her brains sucked out through a straw, before being taken away to be eaten by his pet Peacekeeper zombies, or Xarai as he calls them. Nice. The other Chiana escaped though, leaving her twin to die from loss of brain and being eaten.

Crais = 1 death?

 Episode 3.21 - Wolf In Sheep's Clothing
It seemed to be the only way to destroy Scorpius' wormhole tech - destroy the command carrier. With Talyn aboard, Crais and Talyn starbursted inside the confined space of the hangar, imploding the carrier around them. Starbursting in a confined space though seemed like it would cause their deaths, although we haven't quite seen yet. It sounds like they're dead, but I'm not so sure.

Crichton = 3 deaths

 Episode 1.12 - The Flax
When the transport pod needed to be vented, Aeryn killed John with a lethal injection, having the antidote at hand (even though he was dead, somehow there was an antidote to this specific method of death - handy). However, the antidote got smashed and Aeryn administegray CPR.
 Episode 2.06 - Picture if you Will
A portrait foretold him being electrocuted, and so Zhaan electrocuted him. This was remedied because it was all a plot of Maldis's and he was taken to Maldis's realm, which Zhaan managed to get everyone to escape from.
 Episode 2.15 - Won't Get Fooled Again
While being tortugray by a Scarren, Scorpius's neural clone in Crichton's head caused his heart and brain functions to stop for a few seconds (technically killing him). A few seconds later the kind chip started them back up again, after the Scarren was suitably distracted so Crichton could kill it.

D'Argo = 2 deaths

 Episode 2.06 - Picture if you Will
Again, a portrait pgrayicted him being skewegray by a sharp object - presumably his Qualta Blade, but it turned out to be a pointy bit on Aeryn's Prowler which one of the DRDs accidentally shorted during maintenance. Again, he was taken to Maldis's realm, and Zhaan allowed them to escape. Got it?
 Episode 3.06 - Eat Me
Basically like Chiana's, except he went first. Trapped aboard Rovhu with Chiana, Crichton and Jool, D'Argo was twinned by mad insane blokey-boy Karvok, and one of the twins had his brains sucked out through a straw, and then was partially eaten by Karvok's pet Xarai zombie blokes, before being "rescued" by Chiana, who set the body in flames in the Luxan death ritual, and chucked it off a bloody high ledge. The remaining twin however, was used as a sex slave by Karvok for one of his little minions.

Durka = 1 death

 Episode 2.20 - Liars, Guns & Money (2)
Capturing Rygel (again) and no doubt impatient for another bout of torturing our resident snotball, Rygel proved he could actually do something and fried him with some sort of device. Quite how Durka had let his guard slip that much we'll never know (stupid Peacekeeper).

Jilina = 1 death

 Episode 1.20 - Hidden Memory
Shot by Scorpius's forces, she died aboard Moya, but was comforted by Stark.

Maldis = 2 deaths

 Episode 1.08 - That Old Black Magic
Got beat up by Crichton, Zhaan and some guy who I can't remember his name. However, he didn't have the energy to put himself back together, but manipulated matter in order to create Kaiven and her ship and the portrait and get back at Zhaan that way, which leads to...
 Episode 2.06 - Picture if you Will
This time beaten because (1) Aeryn destroyed Kaiven and her ship, a temporary extension of Maldis, and (2) Zhaan beat him up bad. But he's probably just waiting to come back in series 3 (then again he didn't - written after series 3 - but he'll probably be back in 4).

Moya = 1 death

 Episode 2.11 - Look at the Princess (2)
Yes, even Moya's died. The Builders decided that she was too risky to let live if she was capable of producing Gunship offspring like Talyn, and commanded Moya to die (Moya even agreed) so Leviathans could continue being creatures of peace. However, Zhaan pleaded for her life, and the Builders put Moya's life in Zhaan's hands, giving her the responsibility as they could see she could look after Moya.

Pilot = 1 death

 Episode 2.11 - Look at the Princess (2)
See how Moya died above. But basically, when Moya died Pilot innocently died, and when she was revived Pilot was revived.

Rorf = 1 death

 Episode 2.21 - Liars, Guns & Money (3)
Found snooping around the Shadow Depository and interrogated by Scorpius and Natalla, losing an eye in the process. Later on, during his escape with Natalla and Crichton, threw himself in front of Crichton to save him from a blast from one of the Shadow Depository's guards. Needless to say, Scorpius was none to happy about the guard trying to shoot Crichton, and would probably have shot the guard himself had Crichton not got there first.

Rygel = 2 deaths or 3?

 Episode 1.02 - Throne for a Loss
Some Tavlek mercenaries got annoyed with him (quite understandably) and killed him there and then. But the cellmate next door (some guy from the Ministry of Trau I think) decided to bring him back to life so he could pay Rygel's ransom and gain all his followers. Problem - Rygel no longer has any followers. SUCKER!!!
 Episode 1.16 - A Human Reaction
Rygel was killed on Earth and dissected. Problem was, it wasn't Rygel. Damn (hehehe). So this doesn't really count. Or does it? No, it probably doesn't.
 Episode 3.10 - Relativity
After Xhalax Sun had knocked out both Aeryn and Stark, plucky little Rygel decided to bare his teeth and have a go. Unfortunately he was overcome by the Peacekeeper quite easily and ended up being stabbed. Imprisoned together, Stark managed to sort out his wounds and join with him to stop him dying. He later revealed to Rygel that he did die for a short time, but Stark brought him back.

Scorpius = 1 death?

 Episode 3.21 - Wolf In Sheep's Clothing
When Talyn and Crais starbursted inside his command carrier, making it implode, Scorpius confronted John. He seemed to have no reason to live any more - his wormhole tech was going up in smoke, and soon the Sebacean race would be over-run by Scarrens who would enjoy getting him back and using him as their favourite play-thing again. So he gave John the code to his neural bracelet, John returning the favour by giving him his. And then Scorpius walked into the flames. Dead? Possibly. I'm not saying if it's probably or not (he's been the main baddie for 2 series - who'd take over from him? Hopefully not Grazar. But then there are the mounds of other baddies on John's tail).

Stark = 2 deaths?

 Episode 2.17 - The Ugly Truth
Well, if you can call it a death. He accepted guilt for the destruction of a Plokavian vessel, knowing the fate was dispersal. But because of his form, he was able to control his energies so that he may put himself back together again one day in the material realm, and indeed succeeded in that. Jammy sod.
 Episode 3.17 - The Choice
I'm not entirely sure about this one. It seemed Stark stayed behind to look for Zhaan's spirit, but left his mask to be given to Moya Crichton. Seeing as though he can't do much without his mask, is he really still alive? I think I remember hearing somewhere that he dispersed again, which is why he left his mask. Maybe it's just BBC's dodgy cutting. Anyway, I'll know soon whether to count this properly as a death or not.

Teurac = 1 death

 Episode 2.21 - Liars, Guns & Money (3)
Found he couldn't generate enough flame to toast the generator, so decided to shoot himself. Being a Sheyang he exploded and strangely enough hasn't been seen since. Either he's dead or he's too ashamed of not producing any flame, and odds are it's the former (though it'd be funny if it was the latter - what would Freud say?).

Talyn = 1 death?

Sorry it looks like it's copied from Crais' death, but it is. Should be noted that Talyn had been shut down and stuff, but does that really matter?

Xhalax = 1 death

 Episode 3.17 - The Choice
Well, after Crais APPARENTLY shot her in 3.10 - Relativity, it seems he really left her alive. But in any case, coming back to lecture her daughter, she began to feel sympathy for her. But this time at least Crais wasn't such a crap shot, and nearly shot her out of a skyscraper window. Aeryn saved her, but Xhalax said she was already dead inside, and so fell to her death. Probably.

Zhaan = 2 deaths

 Episode 2.06 - Picture if you Will
Again, the evil portrait pictugray her falling from a great height, and lo and behold she only went and fell off Pilot's little (or in fact quite big if you watch 2.09 - Out of their Minds) pedestal to her death. But she materialised in Maldis's realm where she proceeded to beat him up again and make her escape, along with Crichton, Chiana and D'Argo.
 Episode 3.04 - Wait for the Wheel
When someone had to stay with Nyalla's vessel in order to separate it from Moya, leading one vessel to survival and the other to oblivion, Zhaan volunteegray. Having given a lot of life energy to Aeryn and possibly not surviving much longer, she made the sacrifice necessary, separating the two vessels while condemning herself to death.
