TEAM ENTRY FORM INSTRUCTIONS
|
1. Division for all teams will be based upon age
of the youngest player as of December 31, 2003. Proof of age
may be required.
2. All teams, except Coed Softball, must be of the
same gender.
3. Team Manager must complete and submit Roster with
Entry Application. PLEASE PRINT OR TYPE - THANK YOU!
4.
Each player must sign the waiver form.
5. Teams must have a
minimum number of players. |

6. Make checks or money orders payable to the the
Pasadena Senior Center. Please follow the instructions in the Entry
Application for Credit Card payments.
7. Registration Deadline:
June 2, 2003 (postmarked).

"For, and in consideration of, the Pasadena Senior Olympics
sponsoring the above-described sports program, I/We agree to release, and
forever discharge, the Pasadena Senior Center, California State University, Los
Angeles, and the Cities of Anaheim, Glendale, Pico Rivera and Pasadena, its
officers and employees, from any and all liabilities, demands, or claims for
loss or damage resulting from any injury or damage I/We may sustain by
participating in the above-mentioned activity."
