Quality Survey

Each topic area has a list of 10 statements for your consideration and evaluation. First, read each statement carefully, then at the end of each statement write the number you perceive best represents the degree to which you agree with the statement. There are no wrong answers. The degree of agreement ranges from number 1 (Strongly Disagree) to number 6 (Strongly Agree).

Use the following scale:
