Send an email note to [email protected] with all of the following information.  If you are registering more than one youth, you only need to provide items 4 - 7 once.  PLEASE USE THE EXACT FORMAT REQUESTED.*
 
1.  Name of youth being registered -- first & last name.  (No middle intial.)

2.  Date of Birth of youth being registered -- in this format --> mm/dd/yy . (Two positions for each portion of the date, separated by slashes.)

3.  First Time Indicator -- Yes or No, whether this is the first time this youth has attended this event.

4.  Mailing Address, including street, city, state & postal zip code.

5.  Telephone Number -- in this format --> (xxx) xxx-xxxx .

6.  Name of parent or adult guardian who will be accompanying youth(s) to the event.  (A parent/guardian may accompany more than one youth.  You only need to provide the name of one parent/guardian with your registration.  This person will be signing the form.  If someone different accompanies the youth(s), we'll just change the form at the event.)

7.  Email address you want your confirmation email sent to.  (Your email address will not be used for any other purpose.)

We will send you an email reply confirming receipt of your information.  If we have already reached the maximum number of participants, we will inform you of that.  If you do not receive a confirmation or reply via email within a couple of days, your registration was probably not processed and you should contact us again asking for status.

We will print your information on a paper copy of the registration form and bring it to the event.  It will be filed under the name of the first youth listed in your registration information.

You will need to stop by the registration/check-in table at the start of the event to sign & date the printed copy of your registration form.

*The format you use for supplying information is important to us because we use the information to compose registration forms and for entering into a database.  We use the database information to compile statistics that help us better organize the event.  For example, knowing how many youths are in particular age ranges helps us ensure we have an adequate number of age-appropriate door prizes.
