| MTCU Release Consent Form |
The simplest way to find out what Certifications your workers have or don't have, is to submit the following form:
Download the MTCU Release Consent Form (PDF file)
Have your worker enter his name and SIN, then sign and date it. Enter your company name and address and fax the completed form to your local MTCU office with an accompanying letter asking for Certification information for that worker. You will receive an official transcript back with all registrations or certifications on file. |

- review workers to see what qualifications they may require in their duties
- check certificates currently held to make sure they are not expired
- advise your Contractors to do the same with their employees
