Below is a list of forms required for participating in the Nova Scotia Tri-Service Cadet Display Team. This forms are in the PDF format and can be viewed and printed with most Web Browsers. Notes

CF51 

The CF51 is only required if not previously passed in. This is a downloadable copy of the CF51. If you have trouble downloading the file, you can obtain a copy from any corps/squadron Administration Officer. Note that there are two areas that the parent/guardian must sign and the cadet must also sign the CF51.

Cover Letter

The cover letter provides an introduction and information regarding cadet participation in the Nova Scotia International Tattoo.

Permission Slip 

The permission slip must be printed off and signed by the parent or guardian of the cadet.

Letter to the Teacher  This is a letter to the cadet's home room teacher requesting the early writing of exams, if required.
Medical Information  This is for medical information and must be returned in addition to the CF51.
Contract of Conduct  This is included for reference only. The contract of Conduct need not be signed at this time. It is provided for information so that the cadets and parents understand the rules in place for cadets who are members of the Cadet Display Team.
