Just save the form to your computer and email it as an attachment. You will still have to mail in the check or money order. The cost to register as a blind golfer for the tournament is $150.00 CAD

Please make checks or Money Orders payable to the Nova Scotia Blind Golf Association.
Entry fees and forms must be received by May 15th, 2009
