Programme: Assessment program in EDR.
General objective: To test tools for assessment.
Specific Objective:
· To test all tools and guidelines in respective fields.
· To collect feedback for further consolidation of tools for the assessment
General Background
With the help of AC-Nielsen, Research expertise partner tool test workshop was conducted in Ilam. District Health Office Ilam and BNMT was involved in the tool test process. Facilitators from RHD, BNMT and AC-Nielsen facilitated the workshop.

1. Information collection tools for DHO/District Hospital (HF-1)
2. Information collection tools for PHC/HP/SHP (HF-2)
3. Information collection tools for drug retailers (HF- 3)
4. Information collection tools for private clinics/ nursing home (HF-4)
5. Information collection tools for NGOs (SP-1)
6. Information collection tools for CBOs (SP-2)
7. Household survey form (HH-1)
8. FGD guide for community people - males/females (FGD-1)
9. FGD guide for traditional healers (FGD-2)
10. FGD guide for local committee members (FGD-3)
11. FGD guide for FCHVs/TBAs (FGD-4)
12. PRA guide for district site mapping (PRA-1)
13. PRA guide for village mapping (PRA-1)
14. Guide for data collectors (G-1)
15. Guide for local supervisors (G-2)
16. Check list for quality of service (QA-1)
Feed backs, suggestion and remarks were collected for all tools that will definitely make tools better in form.
1. Physical contacts of senior officials with RHD would not be replaced by just establishing a contact person's/ focal person's office in RHD. (Since, some time a physical contact can solve the crisis, which can not be solved by several letters, telephone calls emails and messages and also chances of communication gap and misunderstanding may be prevented).
2. Adding the number of partners should not decrease role of a partner. Mutual understanding should be kept maintained and strengthened. And, problems and constraints of a partner or person should be understood as well.
