1.	You must have had no felony convictions within the three years prior to applying for this rotation.
2.	Non-U.S. Citizens must provide proof of Alien Residency or a work visa.

Rotation Preparation
Phase 1
Upon being matched to this internship rotation you will need to do the following:
1.	Complete and submit the online Rotation Enrollment Form.
2.	Attend a two-hour orientation class. You will be notified regarding the date and time once your name has been added to the roster.

Phase 2
Two months prior to your rotation you will need to do the following:
1.	Complete and submit all the required application forms.

Phase 3
Two weeks prior to your rotation you will need to do the following:
1.	Review the internship guidelines.
2.	Complete and submit the online Internship Agreement.
