This book is not just psychiatry--assessments cover: depression, neuropsychological tests, neuropsychiatric assessments, activities of daily living, global assessments/quality of life, physical examination, delirum, caregiver assessments, memory functioning, and more!
A new edition is coming out in August 2003; I have the first edition. Each entry includes: the citing reference (usually a journal article), time taken, rating (who/how it's done), main indications, commentary, additional references, and the address for correspondence. Many entries include the assessment themselves--few sources do this. However, to administer them in a standardized manner, you should purchase the assessment kit.
