3) Must be ORIGINAL!
4) Must provide a name. It doesn't need to be your real name; initials or a nickname is fine, but please, don't call yourself Sirius or anything of that like - it only leads to confusion.
5) Please have a title for it, even if the title is just Sirius
6) Include all this (Name, Title, Image, and Thumbnail) in an email to the below address.
