Create or open up the image you wish to use. Mine is below:
Now add a new raster layer by going to LAYERS>NEW RASTER LAYER and hit ok.
Now  go to the style chart next to the color palette and choose the color for your outline and for your fill. Go to the shapes button. Draw a square.
Now go up to LAYERS>PROPERTIES and set your opacity to eithr 50 or 60 or whichever suits you. Tada! Here's what mine turned out like:
