Hold Up, A, and B on controller 2 and reset the NES to access the Sound Test.
At the Sound Test, press B, Up, Up, B, Down, Down, B on controller 2 to enable Stage Select.
At the second title screen after your initial battle in Zawel Castle, hold A and B on controller 2 and press Start on controller 1 to begin with 21 lives.
