EXAMPLES ON HOW TO FILL OUT THE ENTRY FORM
Step-by-step instructions

We have provided several different scenarios on how to fill out the entry form. Choose one that fits your own situation, and fill out your own entry form exactly the same way. Just remember to fill in your own name and address, instead of using the ones provided in our examples. Scenario #1
Entrant is single, and has no children.

Scenario #2
Entrant is married. They have no children.

Scenario #3
Entrant is married. They have one child.

Scenario #4
Entrant resides in a different country than country of birth. He/she is married. They have one child.

Scenario #5
Entrant is born in an ineligible country. But neither of his/her parents was born there or resided there at the time of the birth. Both parents were born in an eligible country, and they resided there at the time of the birth. Entrant may be able claim nativity in one of his/her parents' country of birth.

Scenario #6
Entrant is born in an ineligible country. But his/her spouse is born in an eligible country, which makes the entrant able to claim the spouse's country of birth rather than his/her own.

Scenario #7
Entrant is separated from his/her husband. Since they are not formally divorced yet, he/she must list the spouse on the entry form. They have no children.

Scenario #8
Entrant is formally divorced from his/her spouse. They have one child.

Scenario #9
