Confidentiality Agreement
As
a member of the Transcript Review Team for the
_________________________________ Subcommittee, I agree to protect the
confidentiality of the personal information contained in all transcripts. The
specific details of personal stories shall not be discussed with persons
outside of the Comprehensive Community Assessment process. I understand this
does not prevent me from discussing generalized themes, but limits the
discussion of specific details of a focus group participant’s story to persons
participating in the Assessment process.
I also understand that details of any personal story can only be communicated outside of the Assessment process when there is a signed consent from the individual involved.
