exercise.

find out how many positive lifestyle habits are being practiced. You will say, I am con-ducting a survey for my Health class and I would like to have you fill out this survey
about your lifestyle. Your name will not be used. This is an anonymous survey.

AUDIENCE
Your audience is students in your class and/or their families.

up the questions for the survey.
2. Select twelve or more students for the survey. Report demographic information about the population surveyed (age, gender, grade, ethnicity, ect).
3. Tally the number of lifestyle factors that promote good health being practiced.
4. Look at your results. Using Excel make a table and a graph that shows how many people practice each habit. Which habits do most people practice? Which ones are least
