Yearbook Order Form
This year's yearbook is $20. Parents can be a
Patron for $23.00, a Sponsor for $25.00 or a Benefactor for $35.00. With
contributions of $25 or more, your student's yearbook will be personalized with
his or her own name on the cover.
Print out the order form and order your student a
Nathan Hale Middle School 2001 yearbook. Orders are due by December
6th. Orders can be dropped off in the office or brought to Mrs. Lyon in
room 208.
Checks should be made out to Nathan Hale Middle
School. Thank you for your support and generosity to our yearbook program.
